QTS 1099 Contractor Registration Form

QTS generally uses its own personnel on its projects. However, we do use contract resources on specific occasions for specialized expertise, resource supplementation or to fill out a project team. Consultants who work with us in this way are expected to adhere to QTS' high standards of work quality and professionalism, and must sign a QTS Subcontractor Agreement (this will be sent to you following you application/screening).

Consultants who are interested in working on a project basis for QTS must complete this form as well as submit a resume via email to 1099@QTSnet.com. Note that resumes submitted without completing this form will be discarded.
